Access a form on SharePoint to submit a request.
Upon click on button to submit request for additional name cards.
They will see a form with all the fields filled in :
Name: Username
Position: Position title
Phone number: ******
Email: ********
Preview of the design: front and back. Like a button to select.
(Will require algorithm to automatically select Malaysian design radio button if staff from Malaysia. Their card will show Malaysia address behind, from.sg show sg address behind, those who work.in both countries will have both address.
Number of boxes: # box (suggested number of boxes)
Bottom of the page will have 2 buttons,
Submit to admin | Change details
When submit to admin, it will add to list for someone to print when minimum quantity of users entering details has reached.
If selected change name card details, they will have the same page but with the fields now editable.
If change of job title, it will send to a team for approval.
Can use the same link to send out enquiry to staff for whoever need to refill name cards.
Data from this is to be uploaded and maintained by a team.
