We are creating online courses and as part of the course we want to ask members to fill in an audit document which asks them questions about their business at the end of each module (6 modules). Their answers from each module would then go into an Actions document which outlines what they need to do.
So we need a tool to ask questions in, then export the answers to another tool where we can create formulas to output their required actions based on their answers.
We need to provide a link to the questions within the course modules (using LearnDash for WordPress) which is unique to them so that when they answer the questions no other members can see their data (and visa versa).
They then need to be able to access the file/tool which has their Actions based on the answers to the questions. Again, this file would need to be unique to them.
At this stage we are thinking of using Google Sheets, or maybe Google Sheets along with Google Forms, but not sure if it would work.
*Questions we have*
1) If we create the questions in Google sheets it should then be possible to do all the required formulas and formatting to output the required Actions into another sheet. But we aren't sure how we would make the Google Sheets unique to them. How could we provide the same links to all members within the modules without it showing the previous persons data? If we provide them with a link to a Google sheet which they then have to make a copy of to allow them to edit, would it then be possible to link this copy to another Google sheet which has their Actions in it?
2) We could use Google Forms but some of the questions are quite complex so not sure this would work. Below is an example of the types of questions in Google Sheets, and then the type of output that we want to create in a separate file. Don't think these type of grid questions could be handled in Google Forms?

*What we need*
1) Someone to come up with a solution as to how this could all work. I have suggested using Google Sheets or Forms but happy for someone to come up with another solution.
2) Once solution has been worked out, need someone to set it up and make it all work as we require. This may be creating formulas or macros in Google Sheets, formatting to make it look professional, linking sheets together, making sure the sheets are unique to that member, etc. I would provide the content and structure and more details on exactly how we need it to work and how answers need to be formatted into Actions.
