Required CSS,HTML,javascript,MySQL Administration,PHP freelancer for Intranet project (PHP & Wordpress) job

I have a Wordpress site in which I am developing an intranet. I want a form (HTML/PHP form or CF7 or Gravity) which my employees can fill out and when they submit it, these fields/variables are input into specific areas of text in a pdf/word file. There should be a set of buttons that allow them to view, download or (optional task) send the file to their emails.

I have the Word version of that document and all the places where I would like to input the text filled out on the form are marked in red.

Can you do it? If so, how much will it cost me and how long will it take?

It's a somewhat big project so there's good money in it for the right person. (Native English Speakers highly preferred due to the complexity and need of thorough understanding of the task)
