I was not sure which category to choose for this. It is a test that I had developed to help people determine if they have Avoidant Personality Disorder (avpd).
#1 Please go through the test and determine of it works well, if anything needs to be improved, or changed.
#2 I want to get an email notification when anyone takes the test
#3 I want test results to be stored in a file somewhere so I can access them.
#4 Not sure if it does this right now, but I need some basic info when folks want to take the test liker a name. The option to register and collect info like their email address and phone #
#5 When they complete the test, I want the results of the test sent to the person that told the test, and an option to send the test to someone else to take.
