Need 4 pages : HOME/ABOUT/BOOKING/PLAN

I need an intricate booking page similar to a site for a restaurant with delivery service.

This project is for a cleaning business that allows the site user (client) to select between 3 packages and of those packages the user can select from a list of items up to X numbers each package OR the default package items.

After the customer selects their package, their address needs to be taken and mapped out.
Need a flied for home entry notes and parking notes.
