There is a web page (I will send you the URL in PM) that I need replicated exactly the way it is (IE links work, drop down fields work, etc). I do need one image & two text fields changed.

Once this is done, I will need this hosted for 30 days at a specified domain (which will need to be purchased). I checked & the domain is available & can be purchased through GoDaddy for $1/year.
