Build tool for HTML fields from data

I have an MS Excel Spreadsheet with 68 rows and 40 columns of data. For each row, I would like to have the ability to copy the 40 values and be able to paste them into an HTML form in an automated fashion.

I am not looking for standard data entry. I am looking for a way to copy a row of data from excel and paste it into a series of HTML form fields.

For instance, I would like to copy something like $200, $300, $200, $100 and paste it into an HTML form that has that same number of fields.

If you have any ideas, I would love to help.

I will share an example of the input spreadsheet and also the HTML form that the values needed to be pasted into.
