PHP/MySQL developer to create password protected web-based tool for people looking for speaking gigs

I have a list of 350 associations, meetup groups and networking groups that need speakers. I have identified 25 fields a potential speaker will have interest in before making contact with one of the speaking opportunities. Fields like number of average audience members, event type, frequency of meetings and contact info. Each opportunity will need a notes field so the speaker can add a note that they have attempted to contact the speaking opportunity. Each will need to have its own username (email) and password. I would be the admin and would need a login to add new speaking opportunities and details. I use a 3rd party shopping cart that will take the order but then I plan to manually setup a username and password for each new client. Goal would be to create basic core system for phase I to get some sales to find build of phase II and eventually roll out to national. So please quote as bare bones for phase I and we can work on phase II later.
