Web User Interface Designer

I am in the process of building a web application. Unfortunately, I am not the best user interface designer. I am looking for someone who can work with me on an on-going basis who can take my mock-ups and bring them to life on the web. Your task is not to wire it to the database or anything, but your tasks would be the following:

1. Follow my mock-up designs as best as possible.
   
2. Your design must be 100% responsive
   
3. The user interface must work on Chrome, Firefox, Safari and Internet Explorer
   
4. Put placeholders where applicable
   
5. NOT to use bootstrap, angular or something similar.
   
6. I am looking for a clean and documented HTML5, CSS, CSS Grids, JavaScript and etc..

The objective here is to separate the front-end from the back-end. You will be required to work with me on an on-going basis, so as we integrate the design and if there are issues, your support would be required.

I can share some mockups that I have created once I narrow my search down to a handful of people.

Please DO NOT send me hundreds of your designs, send me only top 3 per the requirements I am looking for. I am not building a website, but an application, with menubars, tables, lists, grids, buttons, forms, maps and etc..
