Need to build an .XL spreadsheet / dashboard for account audit

Need to build an .xl dashboard / spreadsheet for auditing customer accounts.
There are 14 different criteria. Each criteria needs to have a values field, a comments field and an action plan. The 14 criteria are
1. Quality and On Time (value field, comment field, action plan)
2. Claims (value field, comment field, action plan)
3. Injuries (value field, comment field, action plan)
4. Accidents (value field, comment field, action plan) etc etc
5. Time Capture/ Lunch and Meal Breaks
6. Listening Meetins / Corovan University
7. Jobsite Inspections
8. Paperwork Turn In Time
9. Equipment Control
10. Account Profitiability Analysis (This could be a subsection with 20 more values)
11. Safety Awards
12 Other Services
13 Productivity
14. Expectations / Billing/ Processes

Other Deliverable
Charts/graphs will autopopulate when information is updated
Need to be able to duplicate this sheet (Customer 1, Customer 2, Customer 3 etc)
It would be nice to track information per Customer, as well as a total of ALL customers for an average score
Needs to be easy for older non-computer literate employees to enter
Would like it to look nice enough to print out and display.
