Design: PDF download w/calculator for sales funnel

I have a spreadsheet with calculations built, but I need to convert it into a downloadable pdf infographic-style for a sales funnel campaign. We want users to be able to download and play with the input values.

You must know how to build calculations into pdf and control editable fields - the yellow fields in the spreadsheet are for input from the user, everything else should be locked down. Im looking for high quality graphics, design, and a polished deliverable that matches our brand. We will provide logo and colors, but this should be a turnkey download we can easily drop in as a hook in our campaign.

Please factor in time to look at our website and think through layout and design. There is potential for more work - were looking for someone who understands how to build calculators of this nature.
