Edit Website and Create 1 order form, and 1 Checkout.

I need someone to create an order form that will capture a persons name, address, and phone number. They should also create a terms and conditions page (I will provide the information). The form should include a checkbox agreeing to the terms and conditions, and at the bottom a button for payment.Once the form is filled out, all of the information should be saved. The order form will be installed on a subdomain. Also, I need a checkout similar to the one on the website. I will provide an example. I also need current sliders to be edited with new graphics that are better looking.
