I need a database for my employees that are on the job to be able to login somewhere and enter some data, the data will be 3 different options

option 1 - gas - fields will be date, driver on truck, gallons, amt per gallon, receipt picture
option 2 - parking - fields will be driver, location, receipt and maybe more
option 3 - time cards - tbd

i want it to be where after they submit this info, it will go to a admin that will verify that this info is accurate, then after they verify this info is accurate i need them to send it to a super admin who can print reports for accounting purposes.
