Developer needed to created online screening tool

Hello,

My company is interested in designing an internet-based screening program that we can link to our WordPress website and send to potential contractors to collect their information and assess their skills. We envision the program will be a simple 4-pages:

-Page 1: Questionarre with fill in the blanks and 3 document upload requirements
-Page 2: Simple text explaining the screening process that will begin on page 3. Requirement for user to connect webcam and mic for recording. Ideally the user would be able to see themselves on webcam and test their mic on this page
-Page 3: User clicks a start button to begin an embedded video, and webcam and mic will simultaneously begin recording. Upon completion of the video, recording can automatically stop or manually stop by pressing a button.
-Page 4: Final submission page. Text to come up after submitting.

The information, documents, and recording would then need to be forwarded either to our email, google drive, or something similar. We will be able to provide more details upon acceptance of this project. We are open to ideas for what platform this will require (ideally not a subscription platform).
