Export Salesforce notes as a CSV file

We have Salesforce contacts and leads that contain associated notes.

We need to download all the notes into a CSV file along with the contacts or leads ID number (a custom field) so that we may import that notes into another system. We need each notes date, title and content.

This is a one-off project, so the process does not need to be pretty or especially user friendly.
Developer will be provided access to a development sandbox and expected to build an outbound change set so we can move the project into production.
