I am looking for a service on my website that allows clients to book in home services (repairs, installations).

I would like clients to input their postal code/zip code to ensure we service that area.
I would like there to be prompts such as (for example)
1. Which phone do you have?
2. which model
3. what colour
4. issue?
5. etc

then the system will spit out a price for the service...

then the client can choose our available booking.
perhaps we can offer 2 options for payment, pay now or pay on site.
