Graphic designer needed for simple medical referral pad design

Looking for someone to design a simple 4x6 referral pad for a medical clinic. Patient Name, our address and logo, and a few checkboxes. Logo and source files all available and guide document ready.

Would want to see 2-3 mockups trialing different layouts and receive .pdf and source files as a final product. Turn around time 2-3 days, this shouldnt take long!

Please include a couple of samples of similar work when applying.
I will share some example with shortlisted candidates.
