Adding a review page to my Word Press site

Im looking for a detail oriented freelancer who can add a review page to my WP site:

My goal is to ask doctors visiting my website to answer five questions to review LOCUM TENENS AGENCIES (i.e. staffing agencies) that they worked with.

Details:

1 - Site visitor sees a list of 25+ companies with their respective reviews

2 - User can see previous reviews and can submit his own

3 - Existing reviews should include total average score in each category and visitors should be able to see each individual response

4 - Each review has 5 items total:
One Dropdown or multiple choice question
Three items with 1-5 star rating
There should be an average score for all three responses
There should be total average score
There should be a free text bar where visitor can type his comments

5 - the companies that has not been rated yet should still be displayed and offer the visitor to submit a response

6 - I should be able to add questions or company names to the list later on

You are free to create the review using any WP plugin if needed.
