Build a custom dedicated job application website & candidate database

Multipart project:
I have a need for someone to
(1) build a custom website to enable applicants to apply for jobs and communicate their credentials and certifications.
(2) Develop a simple DB (MySQL or similar) that would store the applicants answers securely
(3) Provide a backend admin view that would enable an admin to select applicants filtered on requirements.
(1a) Include a generic captcha gate to limit spam
(*4*) Nice to have: an email notification to a specific e-mail address when someone submits an application.

Note: besides the normal fields in a job application, the specific certifications would include:
7 separate checkboxes
17 different drop-down boxes

This project would include the UI/UX design and templates; but this is not expected to be exceptionally elaborate or complicated. A single webpage for submission would be acceptable.
