Create a form for our wordpress site

We want a fairly simple form for our Wordpress site.
I dont want you to use plugins if possible, much preferred to have code we can use like those examples.

Fields for form:

- Name*
- Business Name*
- Email Address*
- Logo upload (1024 * 1024 recommend but not essential)*
- Background image (1080 * 1920 upload)
- App Brief (Document Upload)*
- Website link
- Facebook link
- Instagram link

The form should go to support- subject New App Build Details.

With the file uploads its obviously important that these are only certain file types and under a certain file size, to avoid malicious uploads. The images would be jpg, png, gif. The docs could be doc/docx/pdf/odt etc.

Not sure exactly what file size limit wed need, but likely no larger than 10MB would be necessary unless you have a suggestion.

Theres an * by each of the fields which need to be required. The others can be optional.

We plan on putting this form on a page on our WordPress site which will not be indexed or accessible anywhere on our sitemap - its something well send to users who purchase our product.
