Create web form with analytics

I want a few Online Fact Find web forms created with specific questions that will always remain the same.

Id like it set up so that I have a few links (each specific to different question sets) that I can send to my clients. They can then put their details in and answer the question set.
Once they put their details in, I want it to create the lead in the admin side so we can track.
The fact finds will be data collection.
When a client adds their assets, I want it to enable them to add asset which will open up another line item for the client to fill in.
I would like a counter that tracks the percentage which is completed as the client goes through for both the client and the admin team to see.
Id like to add a section at the end of the fact find where the client reads our policy and then is asked to sign (either by finger drawing on the screen or with their mouse on the computer)
We need to have this set as printable to be kept for each client.
Some of the answers will need to recorded in a seperate section for the admin team to use straight away.

I have an existing Online fact find that I can show the tasker to make it easier to understand.

We would like to get this made ASAP so please let me know your thoughts and any timeframes you might have.
