Developer for small project in xcode uisng swift

Im looking for an ios developer for a small project according to following description (please also take a look at attached mockup). The delivery should be in a form of view controller scene and swift files that can easily be added to an existing project. As needed, instructions will be provided as well.

Requirements:
compatible with xcode 10.1, swift 4.1 and above.
