Fix max connections SQL error

Recently I started getting max connections error and I have to restart my server in order to fix it. The things I tried are turn disabling persistent connections, increasing max connections it just maxes it out and lags my site, and decreasing wait_time variable no dice. I think there is a script problem or a problem with my sql server need fix asap.
