Integrate Supplied API to Service Provider

We need to create a single web page (to be added to our website that is already up and running) that would use an API (That will be provided) to link to our service provider that will enable clients to signup for an internet service.

In short - I have an API that needs to be used to connect to a service providers systems to place orders online.

I need someone who can create this single page for ordering and then intergrate the supplied API to the service provider.
