Layered Process Audit App

Looking for someone to build me an app to complete process audits for a manufacturing environment. This app will ask simple questions and have the ability to report a finding that create a open issues list and send out a finding email. The open issues list will be a running log to give people the ability to revisit and close.

There are many software packages out there that can do it but I dont want to pay per user.
