Need a 3D designer with access to a 3D printer for a simple product

Im looking for a 3D modeler with access to a 3D printer to do the following tasks:

1) Spend a bit of time with me (on the phone and TeamViewer) looking at something I sketched out on a piece of paper

2) Take my sketch and other information provided in above call and make the required 3D model

3) Print the components in a 3D printer

4) Review the resulting product with me over a video chat

5) If no changes are necessary, send me the printed out components as well as the 3D design file.
