pdf annotator

I need a webpage that I can access after login.

After login it should look like this

it should show course name, assignment name, and view all submission
in the middle it should show student name and email
on right side it should show all students in drop down

then it should show pdf annotation tool bar, which I believe is taken from yui library .
in the middle it should load pdf for annotation as shown in this picture, On the right side, it should allow saving marks for each question and then saving comments

this is taken from moodle where student-submitted work is graded by a teacher. I need to embed this feature in my customized LMS.

I will share some documents with shortlisted candidates for better understanding.
