Ruby On Rails Server Issue - Need urgent resolution

Need assistance from the developer to resolve the memory issue we are facing with our existing website which is built in Ruby On Rails.
We have found out that the memory usage of our server on AWS keeps on increasing daily. After 20-25 days the memory reaches 100% and we have to restart the server.

Ubuntu 14.04
Ruby 2.5.1
Rails 5.2.0
Puma 3.11.4

Memory starts at around 20-30% after server restart. We run delayed jobs which take 15% memory but this usage kind of remains constant. Puma memory usage keeps on increasing gradually.
