Setup MySQL table + Grocery CRUD

I have fresh server (on digitalocean) with setuped and working LAMP.

I need somebody who will setup on my server following things:
- setup MySQL table (using phpmyadmin) (I will provide table structure, it will be only one table with around 70 fields)
- install grocerycrud to manage this table
This is free frontend library available here https://www.grocerycrud.com/downloads
(I want to have bootstrap V4 theme like on example here https://www.grocerycrud.com/demo/bootstrap_theme, just working with my MySQL table)

I will provide access to SSH, perhaps it can required to enable or create FTP account (or you can download all needed stuff by SSH - up to you).
After finished task I will need access data to all things.
