I need javascript to allow PDF form to auto fill &lock fields depending on the password entered

I need javascript to allow PDF form to auto fill &lock fields depending on the password entered.

I have a fillable form in PDF, on push button click it will permit the user to enter a password to lock and unlock certain fields.

I need two things,

1. a button with javascript, with 10 passwords that can be entered to lock the fields, and once a password is entered to lock the fields it will only unlocked by the first password entered to unlock

2.another button with javascript with 10 passwords, once a certain password entered a predetermined data will fill a certain different fields (Signature fields: Employee name field, Employee ID number, Current date, Current time) . and can only unlocked by the password first entered.

Chose or design 2 different business style button images
