I need someone to create a pdf form that saves to template xml for import to cad program

I need a form created, preferably a pdf, that can be used to export many of the form fields to a template xml file that I will import into my cad program to populate autotext fields in my new projects.

I will provide xml file is the one exported from my cad program and the form will need to populate many of the fields so that I can import the data back into my cad program. For this sample I have identified the fields that need to be imported and which ones are not used, or are static (fixed value).

The purpose of this form is two-fold. I intend to have my assistant gather the pertinent data and enter it into the form for 1) hard copy printing as a project data sheet, but most importantly, 2) to be used as the project data source for populating the template xml file so that my cad tech can import the xml file into new projects. This imported data will then auto-populate text entries throughout the project layouts inside the cad program.

I want the form to be easy to use and easy to save/export to my cad xml template.

The printed form needs to be formatted and organized neatly because the hard copy will stay with the project folder as a reference file for project, site, and client data.
