Building web hosted/app for short survey with dynamic calculations based on previous answers to questions that is easily shared by link

This would work hopefully like a very straightforward 15-17 question SurveyMonkey survey with a link to complete it with literally only two calculations in one of the questions. It is a one-time survey currently. Unfortunately none of the survey platforms offer this functionality which is why I am looking for something bespoke. I have a very clear mockup available on request so should just require implementing
