Required Graphic Design,3D Design freelancer for Format Book for Amazon Print on Demand job

I have a book and I will provide all the text and copy.
I need it to be formatted into a Book for Amazon Print on Demand. Since this is my first time taking on such a project, I am not familiar with all the steps of the process. As I understand it here is how it goes:
1.Design Cover: Need to be done by freelancer
2.Format Book Layout: Need to be done by freelancer
3.Insert Copy in Layout: I will do this
4.Typeset the text for printing: Need to be done by freelancer once I insert copy and adjust.

For now I would like to get steps one and two done. I would need a freelancer that has specific experience in all of this.

The book is for dentists and is a business to business book.

The Book Could be Titled: The Ultimate System to Increase Case Acceptance for Dentists
