You did some work for me a couple of months ago and I would like to offer another project to you.
There are two components to this project.
Component A - Digital PDF with Scanner
I have a PDF which has editable fields. I need this to be a little more functional and also include the ability to upload a photo or integrate with a scanner so two documents can be added to the form.
How the form works is that two staff members attend a job site. They can complete the first half of the job sheet once they have arrived onsite. There is a single piece of paper that is a government form they fill out (pick-up form). They give one copy to the attending police officer, they keep one to give to the receiver and we get a copy. Instead of them posting the hard copy to me, I would prefer they could take a photo or a scan image and upload it as an additional page to the PDF Form.
When they get to the receiver destination they then can complete the other bit of the PDF. They will also get another form with is called the delivery form. Again I would like this form scanned and uploaded to the PDF.
I then need them to be able to send me a copy of the PDF via email or file upload.
Component B - Collate information in excel database
Basically I need the information from the PDF they have filled out to be put into an excel database. I will have a few different columns but most of them will be what is pulled from the PDF.
Please let me know your thoughts and if it is something you can do.
