Required Visual Design,Mockup,HTML freelancer for Webpage Design (Simple) job

Hello!

My studio is currently working on producing a worldwide MMO. We are in need of simple web pages for the following pages:

Email Verification (leave space for recaptcha)
Forgot Password (Text fields for new pwd and new pwd confirmation)
Simple HTML for a email sent to verify their email address

The game is titled 'Trials of Titan' and I have included some assets you may use to display on the page. 'Unnamed Studios' is the studio name and I've included the some logo assets. My ideal page would be simple and easy to use. Also displaying the studio logo and game logos would be nice. I've also included an image of what textfields look like in-game, so you may mimic the style.
