I have a PHP file that is supposed to add entries into my localhost MySQL database. The issue is that although many of these entries are correctly entered, there are some that simply do not get entered. After trying to identify the culprit, I check PHPMyAdmin, and found that I have what seems to be a very high 'Failed Attempts' amount. Here is the stats:
