Required eCommerce,Database,PHP,API freelancer for Secure PHP Ordering form may include REST API, SSL or AJAX job

We work with a eye wear company in Hong Kong, ordering eyeglasses and lens with a prescription. Here is the current ordering process: (removed by Toogit admin) (start at :50). It would be a conditional javascript form (with auto suggest) that would autosuggest options for each field from a database (example: frame color: red, blue, green etc), after the fields are filled out, the submit button would 1) enter the order in a database 2) possibly connect to a third-party for ordering. Let me know of any additional information needed.
