I run the file through a software designed to reduce background noise and improve audio quality. However, if I am unable to make out most of the conversation even after cleaning the file, I make this known to my client.

I do my best to provide accurate Transcripts the first time to avoid wasting the client's time on revisions. But in case of anything you can request revisions until you're happy with the work.
