Required Data Entry,Microsoft Excel,SurveyMonkey freelancer for Need a person to help establish some survey monkey documents job

We need to complete annual skills checklists for our employees. We want a document that can be emailed out to our staff members and then have them return it so it can be saved into there file. This is basically a multipage checklist of various skills that the employee rates themselves based upon frequency and competency. I need it completed within the next 7 days. I have PDF examples of what we need and I have a survey monkey account created.
